    The beginning of the end of vet school

    As regular readers of this blog may have noticed, I was a little apprehensive about starting my final year at veterinary school…

    Having already been in the small animal hospital for two days, we finally received our results – confirming I and many of my fellow classmates had passed our exams and could now wear our final year jackets without guilt and walk around the hospital without feeling like imposters.

    However, despite now knowing we had qualified to be in the hospital, it still felt like we had been thrown in the deep end.

    In at the deep end

    My first rotation was emergency and critical care, with the first part being internal medicine. The first couple of days were spent frantically researching the background of patients coming in for appointments, bumbling through clinical exams and brushing up on my rusty practical skills.

    It was my first time taking consults alone and, after missing out key questions the first few times, I eventually got into the swing of things and made fewer mistakes.

    Despite feeling like I didn’t know anything to begin with, I at least managed to scrape together a few sensible ideas when clinicians tried to worm differentials out of us. It has been a steep learning curve, changing the way of thinking entirely to apply things to a real patient in front of you, which usually has not read the textbook.

    OOH my goodness

    Just as I was beginning to feel comfortable with medicine, we swapped to out of hours – which, against my presumptions, turned out to be a really enjoyable week.

    I adjusted to nights far easier than I expected and was powering through until one particularly long night when a bulldog came in with a suspected gastric dilatation volvulus (GDV).

    This was the first genuine emergency we’d been involved in and stress levels were running high. Having rapidly set up fluid boluses, taken radiographs to confirm our suspicions, checked lactate levels and run in-house bloods, we went through to theatre. After a very long night of surgery and having warned the owner of an extremely grave prognosis, we were delighted to see said bulldog looking bright and happy the following evening, eating and pulling us down the corridors to the runs outside.

    Not all GDVs end with such a happy ending, as we had learned earlier in the week – a dog that underwent the surgery at its own vets came to us for overnight care in ICU and, after a rocky night of a supraventricular tachycardia that we struggled to keep under control, crashed the following morning, was resuscitated successfully once, but could not be saved when it crashed again minutes later.

    Hearts, not brains

    Coming from nights straight back into days, however, was much harder and I felt like a zombie for the first day of my cardiology week.

    On the subsequent days, when my brain was working again, I was able to make a bit more sense of echocardiography and gain a better understanding of some conditions and the tray menu options available.

    I also learned a bit more about the genetics of Bengal cats and found trying to heart scan a cat that’s only two generations away from a leopard cat can be quite challenging (and may involve chasing said cat around the ultrasound room for some time, following an artful escape act).

    This year isn’t going to be a picnic, but, although I already feel exhausted, if last month is anything to go by, it will be an enjoyable one.

    I graduated from the University of Liverpool in 1988 and completed a PhD at the Rowett Research Institute in Aberdeen. I continued my research interests and pathology training at the University of Edinburgh’s Royal (Dick) School of Veterinary Studies, becoming a senior lecturer in veterinary pathology.

    Pursuing an interest in human health research, I moved to the pharmaceutical industry in 2001 to work as a pathologist in the safety assessment of new medicines and managing a large technical team. While in industry, I maintained strong academic links, with particular interests in transferring technical skills between industry and academia, and encouraging vets to explore alternative career options.

    In recognition of these interests, I was awarded an Medical Research Council (MRC) skills gap grant, moving back to the RVC in 2009 and on to my current role as pathologist at MRC Harwell in 2012. I am an RCVS recognised specialist in veterinary pathology and hold visiting chairs at RVC and the University of Surrey.

    I have experience of serving on and chairing professional committees. I was part of the stakeholder group for the Vet Futures project and chair the Royal College of Pathologists’ specialty advisory committee for veterinary pathology.

    Manifesto

    The profession is undergoing modernisation and facing challenges that require new ideas to resolve. Key to development and innovation in any profession is the cultivation of diversity. Diversity includes the individuals within the profession, working practices and career pathways.

    Increasing diversity means, firstly, looking at our student intake, working with schools and aiming to attract a better balance of applicants that reflect our society today and will make resilient vets.

    A veterinary education provides a great starting point for a variety of potential career pathways. The Vet Futures project has shown the RCVS and the BVA recognise different career options are important for individuals and the professions. We need to build on this project to ensure graduates are prepared for different roles, practice and non-practice career paths are equally valued, career paths are signposted and people are supported throughout development in their chosen career.

    Mental health is another pressing issue for our profession and ensuring a satisfying career choice, with manageable levels of stress, are essentials for good mental health. We need to develop career pathways within the profession that offer progression and working practices that accept a good work-life balance is essential, not just desirable. We need to find ways to support vets through their early years and as they develop greater responsibilities later in their career.

    Diversity in the profession means having a range of resilient veterinary graduates able proactively to engage with a range of different career options and to adapt to changes in the external environment. Having worked in the university, research institutes and pharmaceutical industry as a lecturer, mentor, careers advisor, researcher and manager, I believe I have a range of skills and experience to contribute to the debate in these crucial areas.

    I graduated from the RVC in 2004 and spent the following two years working in a busy small animal hospital in Bristol. Having been firmly bitten by the medicine bug, I returned to the RVC in 2006 to undertake an internship and, subsequently, a residency in small animal medicine.

    Following completion of my residency, I returned to the west country to work for the University of Bristol and Langford Veterinary Services as a teaching fellow and internal medicine clinician. I worked at Langford for a very enjoyable two-and-a-half years, before taking a short career break to help to establish the charity Brighter Bristol.

    Since qualifying, I have also had experience of working in the veterinary charity sector and private referral hospitals, both in the UK and US. In my current position as an internal medicine consultant for Axiom Veterinary Laboratories, I am able to combine my enjoyment of veterinary internal medicine with a role that allows me to offer support to practitioners with their cases.

    Outside of work, I love to travel and enjoy being creative. I also continue to volunteer for a number of charities in the Bristol area.

    Manifesto

    Like all vets, I have worked hard to become a member of the RCVS and am incredibly proud to be so. With ongoing challenges facing us, these really are exciting times for our profession and, as members, we should be engaged with our college to help create and shape our preferred future.

    Having worked in a variety of clinical and academic settings, it is of no doubt to me a happy, healthy vet makes a good vet. As a role that has significant emotional and physical demands, it is no wonder a recent survey of BVA members reported reducing stress was the single highest priority for many. I believe there needs to be a culture change within the profession to support mental health and well-being, and the launch of the RCVS Mind Matters Initiative must be applauded as there is much work to be done to provide support to our members.

    Veterinary education needs to meet the demands of the modern profession and it is essential the college reflects and promotes all aspects of veterinary work. It is alarming half of the vets surveyed who graduated within the last eight years reported their careers had not matched their expectations (reference: Vet Futures). Action must be taken to ensure we meet their aspirations and do not lose the skills of these highly qualified individuals.

    I am passionate about the advancement of knowledge within veterinary science, particularly making use of evidence-based medicine and clinical governance. With a number of groups promoting these in practice, there is great potential for input from all areas of the profession.

    If elected, I promise to contribute an enthusiastic and progressive voice to council and I would consider it a privilege to serve the veterinary community in this way.

    Since qualifying from Bristol in 1980, I have worked in a wide range of first-opinion practices (mixed, pure equine and pure small animal) and, for seven years, was involved in the tuition of final year veterinary students while working for the former Glasgow University Practice in Lanark. I achieved the CertVR from practice in 1991 and obtained an MLitt for archaeological research in 1989.

    Since 1994, I have run a small animal practice in Carlisle with my wife Kirsty, which has grown to employ five veterinary surgeons and four RVNs, and accepts student VNs from three different colleges. The practice achieved the Investors in People Award on three occasions. I take primary responsibility for cardiology, imaging and orthopaedic work.

    Beyond work, my interests include skiing, cycling and DIY, keeping my English setter fully exercised and watching cricket.

    Before offering myself for election to the RCVS, I spent two years on SPVS Council; I was elected to RCVS Council for the first time in 2012.

    Manifesto

    I have been fortunate to be a member of the standards committee throughout my first term on council. Currently its vice-chairman, I have sought to ensure demands made of practitioners are both reasonable and deliverable.

    I was part of the post-Chikosi 24/7 review, which, we hope, has more clearly defined the responsibilities of both owners and veterinary surgeons. Another review may prove unavoidable as consumer choice leads to the fragmentation of veterinary service provision, with resultant confusion as to who exactly carries responsibility for the emergency care of those animals “registered” with more than one supplier.

    A recent survey, part of the Vet Futures initiative, has revealed worrying levels of disillusionment among younger members of the profession. Further analysis will, I hope, help us to understand the reasons for this. While dedicated emergency services have improved work-life balance for many, as 10-hour days become common have we simply changed the nature of work-related stress? I find it sad fewer graduates now express an interest in running their own practice; while it is undoubtedly challenging to run your own business, you can at least take control of your professional life. If this trend continues then the number of genuinely independent practices will inexorably decline.

    Next year should see the conclusion of the current review of RCVS governance, a review likely to lead to a significant shrinkage in the size of council. Elected members will remain in the majority, but their numbers will be reduced. It will be important council retains members with relevant and recent experience of first-opinion practice. In this regard, I believe I can continue to make a worthwhile contribution and would ask for your support in this election.

    Mega dairies: intensive farming? Or just efficient?

    Delegates who participated in the farm stream at the 2016 Association of Veterinary Students (AVS) Congress in Liverpool (myself included) had a visit to a so-called “mega dairy”.

    The herd we visited had around 1,000 milkers, which seems large when compared to the UK dairy herd average of 123 (according to AHDB Dairy). Yet, in the grand scheme of things – when considering the real dairy giants in places such as the US with herds ranging up to 30,000 and the proposed 100,000 cow dairy in China – it’s questionable as to whether this farm could really even be considered “mega.”

    Putting numbers aside, the real reason we were taken to the farm was to see how these large-scale, high-intensity dairy production units function.

    Preconceptions versus reality

    The public shudders at the words “intensive farming” and, as a vet student, I like to think I have a slightly more informed view on what that means. Saying that, I hadn’t been to a dairy farm on such a scale as this before and was pleasantly surprised by the forward-thinking approach to every aspect of dairying.

    The concept behind many of the methods used on this farm is based on maximising production by reducing stress levels in the dairy cow, thereby minimising any health implications and reduction in yield. This is done by time budgeting by accounting for time she wants to spend feeding, lying down, eating, drinking and socialising. The “leftover” time is used for milking, with management tasks fitted around – i.e. pregnancy diagnosing during her time at the feed face.

    Interestingly, the zero-grazing system allows for the cow to regulate her own feed intake depending on her stage of lactation. The straw-based total mixed ration fed is the same for every cow, and differences in energy requirements rely on the reduction of rumen size during late pregnancy to sufficiently reduce intake. This minimises fat cows during late lactation and drying off.

    Digital motility assessing scales are used to pick up any load imbalances, which can then be addressed immediately, opposed to 2-3 weeks later, when the cow would begin to show clinical lameness. There is also emphasis on the milking parlour being a stress-free environment so the cattle can enjoy being there without the need for food bribery.

    Efficiency effects

    Maximising efficiency is key, not only in terms of the cow, but the unit as a whole. Water used on the farm is recycled five times by using it for drinking water, washing and filtering bedding sand from slurry. This process also allows the sand to be cleaned, dried out and reused for bedding. Large solar panels on the roofs of the open-sided housing sheds provide more than enough electricity for the whole unit, with the surplus being shipped out to the surrounding area.

    Research has shown the optimum environmental temperature for a cow is much lower than perceived – this is accounted for by the huge open-sided housing buildings and ventilation system. Each detail seems to have been meticulously considered, with continual monitoring and data analysis taking place to ensure production efficiency is maximal. In the efforts to reduce antimicrobial use within the veterinary profession, the farm’s claim to have halved its use, and any veterinary involvement in the last few years, can only be applauded.

    With its huge open sheds, cows bedded on a calculated depth for maximal comfort with access to feed at any time and constant monitoring for early signs of stress or ill health, while not the idealistic picture of a cow moseying around a field at her leisure, was not far off, and certainly not the grim jumped-to conclusion when it comes to intensive farming.

    Public perception is always going to be a huge hurdle for farming, but perhaps we can change that through the right sort of publicity, starting with abandoning the phrase ‘intensive farming’ and all associated misconceptions, and replacing it with something like ‘efficient’ or ‘environmentally friendly’ farming, because that is exactly what it is.

  • Interview nerves? How to calm yourself before an interview

    Of course you’ll be nervous – it’s an interview! Accepting that you will be nervous allows you to work on some strategies to help before and during the interview.

    The Pen is Mightier… I find that many people are helped by the act of writing. I suspect that it’s the act of writing as well as what you write. What to write? If you are nervous about forgetting something, write down a list of what you need. If you are nervous about certain interview questions, then write out your answers. You won’t deliver them exactly as you write, but you will remember most of what you write. If you are nervous about certain behaviors (talking too fast, giggling, not smiling), then in the corner of a piece of paper write a coded symbol that reminds you to smile, listen, etc.. If you are waiting outside of the interviewer’s office, write a thank-you to the interviewer in advance. Writing that thank-you allow you to focus on the successful outcome of the interview. You will drop off the thank-you as you leave the interview and appear well-organised.

    Help someone else. If you need to practice for an interview, find someone else who needs to practice. Use your new partner to practice with and seek opportunities to offer advice. When you realise that your problems are similar and normal, you will relax. Although you may face similar challenges and concerns, you will find it much easier to solve their problems than yours. Along the way you’ll get insights into your own challenges.

    Be your own third party. Even if you don’t have a buddy to work with, consider pretending that you do. Take a look at your situation and ask: “If I were someone else what would I do?” This technique is particularly powerful if you identify someone you feel handles these situations well. If you know that “Bob” is always good at discussing his career, then imagine how “Bob” might handle a tough interview question.

    Find your “peaceful place”. Before an interview consider the place you’ve always felt safe and relaxed and happy. Practice seeing yourself in this place. When you get nervous picture yourself again in this place.

    Concentrate on a detail around you. Nervous people relax when their attention is diverted, so this is a good waiting room strategy. That’s partly why doctors’ offices have magazines. But you can find an interesting painting to look at, or window to study the clouds outside. Focus on details and try to memorise what you see.

    Ask questions. During an interview if you ask questions the interview will become more comfortable for both you and the interviewer. Have a few questions prepared for the interviewer, “How long have you worked here?” Making an interview into a conversation allows you to feel the normal comfortable rhythm of conversation.

    Nervousness is normal. Accept that your nerves might help you stay energised. Then, use simple strategies to manage the stress level.
